These Terms and Conditions ("Terms") constitute a legally binding agreement between you ("Client," "you," or "your") and His Majesty Home Care LLC ("Company," "we," "us," or "our"), a home care agency located at 1275 Bloomfield Ave, Bldg 2, Unit 17A, Fairfield, New Jersey 07004.

By accessing our website, submitting a contact form, or engaging our home care services, you agree to comply with and be bound by these Terms. These Terms apply to all visitors, clients, caregivers, and others who access or use our website and services.

Description of Services

His Majesty Home Care LLC provides medical and non-medical home care services designed to help clients remain safe, comfortable, and independent in their homes. Our services include, but are not limited to:

  • Skilled Nursing
  • Personal Care
  • Companion Care
  • Medication Reminders
  • Meal Preparation
  • Light Housekeeping
  • Mobility Assistance
  • Respite Care

All services are subject to availability and will be provided based on an individualized care plan developed in consultation with the client and/or their authorized representative. The scope, frequency, and duration of services may vary depending on the client's needs and the terms of any applicable insurance coverage or payment arrangement.

Eligibility

Our services are available to individuals who reside within our service area of Fairfield, New Jersey and surrounding areas. To be eligible for services, clients must meet any applicable medical, insurance, or program-specific requirements.

By engaging our services, you represent that you are at least 18 years of age or are the authorized legal representative (such as a parent, guardian, or power of attorney) of the individual receiving care. If you are acting on behalf of another individual, you represent that you have the legal authority to agree to these Terms on their behalf.

Client Responsibilities

As a client of His Majesty Home Care LLC, you agree to the following responsibilities:

  • Provide accurate, complete, and current personal, medical, and insurance information
  • Notify us promptly of any changes to your health condition, medications, or care needs
  • Maintain a safe and accessible home environment for our caregivers
  • Treat our caregivers and staff with courtesy, dignity, and respect
  • Notify us in advance of scheduling changes, cancellations, or if you will not be home for a scheduled visit
  • Provide timely payment for services rendered in accordance with agreed-upon billing terms
  • Communicate openly with our team regarding any concerns, preferences, or feedback about your care

Payment & Billing

Insurance & Medicare: We accept Medicare and work with various insurance providers. Coverage, co-payments, and deductibles are determined by your specific insurance plan. You are responsible for understanding your coverage and for any costs not covered by your insurance.

Private Pay: For clients who pay privately, rates and payment terms will be discussed and agreed upon prior to the commencement of services. Payment is expected according to the schedule outlined in your service agreement.

General Billing Terms:

  • All fees and charges will be communicated to you before services begin
  • Invoices are issued on a regular billing cycle as agreed upon
  • Payment is due upon receipt of invoice unless other arrangements have been made in writing
  • Late payments may be subject to additional fees as outlined in your service agreement
  • Any billing disputes must be communicated to us in writing within 30 days of the invoice date

Cancellation & Termination

Client Cancellation: You may cancel or modify scheduled services by providing us with reasonable advance notice (at least 24 hours prior to the scheduled visit). Cancellations made with less than 24 hours' notice may be subject to a cancellation fee as outlined in your service agreement.

Client Termination: You may terminate services at any time by providing written notice to His Majesty Home Care LLC. Any outstanding balances must be paid in full upon termination.

Company Termination: We reserve the right to discontinue services under the following circumstances:

  • Non-payment of fees after reasonable notice and opportunity to cure
  • Unsafe or hostile conditions that pose a risk to our caregivers or staff
  • Verbal or physical abuse directed at our caregivers or staff
  • Client's care needs exceeding the scope of services we are licensed or qualified to provide
  • Client's refusal to comply with the agreed-upon care plan after reasonable discussion

In all cases, we will provide reasonable notice and assist with transitioning care to another provider when possible.

Our website is provided for informational purposes and to facilitate communication with potential and existing clients. By using our website, you agree that:

  • You will not use the website for any unlawful purpose or in any way that could damage, disable, or impair the website
